This form is for changing the person responsible for billing. It is only applicable to the following and will require:

Death – a death certificate will be required
Divorce – deed showing that the person owning the home has changed
Marriage – marriage certificate
New Management Firm – email from the present owner instructing MMLD to send the invoices for electricity to the new management firm.
